While I understand that I retain all rights to ownership of the work, I hereby give the Shriekfest Film Festival permission to feature, advertise, display, in whole or in part, the work titled and described above. I also understand that my entry is valid only upon completion of the following conditions: (1) receipt of a copy of the work and this completed signed form, (2) the processing of the entry fee by the festival organization, and (3) when the work is found to be original and owned by me or those I represent.

Step 3: Place the completed form, your entry fee check, a self-addressed stamped envelope (if you
would like your entry returned), and 2 (two) copies of your NTSC VHS or DVD entry or 1 (one) copy of your screenplay in a single
package for submission. Screenplays must have a title page with the title and writer's name.
All accepted filmmakers will be required to supply a self addressed stampled envelope for their
screeners and applicable insurance (if needed).

IMPORTANT NOTE: We are not responsible for lost, stolen, or missing entries. We would like to remind you to send a COPY of your original work, not the original work itself. Tapes, DVD's & Screenplays will not be returned unless a SASE is
provided, however, we will contact you to let you know we received your film or screenplay and we will contact everyone whether you are a finalist or not. Please do not send submissions "signature required". We will email you to confirm receipt. We will also provide JUDGES' NOTES if you request them via email in late October.

These same prices apply to the screenplays too.

The format that will be accepted for qualification is NTSC VHS or DVD,
but festival screenings can be on most any format.
